IVF Symptoms Explained: What to Expect at Each Stage of Treatment
Learn what symptoms are normal during IVF stimulation, egg retrieval, and embryo transfer, and when to call your doctor.
In short: During stimulation you may feel bloating, breast tenderness, mood swings and tiredness; after egg retrieval, mild to moderate lower abdominal pain and slight bleeding; after embryo transfer, mild cramps and bloating. See the doctor at once for severe bloating, rapid weight gain or shortness of breath (signs of overstimulation), heavy bleeding or sharp pain.
Going through an IVF program can bring up a lot of questions about how your body will feel along the way. IVF treatment involves several steps, and most rely on hormonal medications and injections to help stimulate the ovaries, retrieve eggs, and support the early stages of pregnancy. It is completely normal to experience a range of physical and emotional symptoms during this journey, as your hormone levels change throughout treatment.
Understanding what to expect at each stage can help you feel more prepared and less anxious. Here is a general guide to the symptoms many patients notice during an IVF cycle.
Common Symptoms During Ovarian Stimulation
During this phase, hormonal injections such as FSH, LH, or gonadotropins are used to help the ovaries grow multiple eggs. You may notice:
- Abdominal bloating or a feeling of fullness — due to enlarged ovaries
- Breast tenderness or sensitivity — related to rising estrogen levels
- Mood swings or emotional ups and downs — often linked to stress, anticipation, and hormonal shifts
- Bloating or gas — possibly from fluid retention and certain medications
- Fatigue or flu-like feelings — sometimes caused by hormonal changes or the daily injections
These symptoms are usually mild and expected as part of the stimulation process, but your care team is always available if anything feels concerning.
Symptoms After Egg Retrieval
After the trigger injection and the egg retrieval procedure, it is common to experience:
- Mild to moderate lower abdominal discomfort — a normal response to the procedure or ovarian stimulation
- Bruising at the injection site, especially with subcutaneous needles
- Light bleeding or spotting, which is generally normal after egg retrieval
- Mild fatigue or dizziness, often related to the anesthesia or the procedure itself
Symptoms After Embryo Transfer
Following embryo transfer, your doctor will usually prescribe pregnancy-supporting medications such as progesterone. During this waiting period, you might notice:
- Mild abdominal cramping
- Bloating or gas
- Breast changes or tenderness
- Mood swings, which are understandable given the emotional wait for your pregnancy test
This can be an emotionally sensitive time, and it helps to know that mild symptoms like these are common and do not necessarily indicate whether the treatment has worked.
Rare Symptoms That Need Prompt Medical Attention
While most symptoms during IVF are mild, it is important to know the signs that require urgent care:
- Ovarian hyperstimulation syndrome (OHSS): severe abdominal bloating, rapid weight gain, shortness of breath, or significant nausea and vomiting. Contact your doctor right away if these occur.
- Heavy bleeding or severe pain after egg retrieval — this needs urgent medical evaluation.
- Allergic reactions to injections (uncommon), such as skin rash or intense itching.
Tips to Help Manage Symptoms
- Apply warm compresses to your abdomen to ease cramping.
- Drink plenty of fluids and reduce salty foods to help with bloating.
- Rest when you feel tired — your body is doing a lot of work.
- Reach out to your doctor if you notice anything unusual or severe.
