PRP (Platelet-Rich Plasma) Therapy in IVF: Supporting the Endometrium and Weak Ovarian Reserve

Learn how PRP therapy may support endometrial thickness and ovarian function during IVF treatment.

In short: Platelet-rich plasma (PRP) is prepared from a sample of the patient’s own blood and injected into the lining of the uterus or the ovary. It is offered to women with a thin lining, a weak ovarian response or repeated IVF failure. The procedure is simple, but results vary from one woman to another and it is still under research.

PRP (Platelet-Rich Plasma) therapy is one of the newer approaches being explored to help support IVF treatment, especially for patients who have experienced repeated implantation difficulties.

Repeated IVF failures can be emotionally difficult, and researchers around the world continue to study the possible causes and ways to improve outcomes. One area of interest is PRP (Platelet-Rich Plasma) injection, a technique being studied for its potential to support weak ovarian function and improve the thickness and quality of the endometrium (the uterine lining).

Below are two of the main ways PRP is currently being used in reproductive medicine.

  1. PRP injection into the endometrium, combined with G-CSF (Granulocyte-Colony Stimulating Factor)

Some cases of IVF failure may be related to the endometrium — either its thickness or overall quality. In these situations, a PRP injection into the uterine lining may be considered as part of a personalized treatment plan.

This is a simple, in-clinic procedure that does not require anesthesia.

A small blood sample is taken from the patient and processed using a specific technique that separates the platelet-rich plasma from the other blood components. This plasma is rich in growth factors that are thought to help stimulate the regeneration of endometrial cells, supporting the development of a healthier uterine lining. When combined with G

As with ovarian PRP, the evidence for intrauterine PRP and G-CSF is still limited: some small studies report a thicker lining and better implantation in women with a persistently thin endometrium or repeated implantation failure, but large high-quality trials are lacking, and international societies (ESHRE 2023) do not yet recommend them routinely. We therefore consider this option only for selected patients after other causes have been checked and treated.-CSF, this approach aims to further support endometrial growth.

  1. PRP for low ovarian reserve or premature ovarian failure

The use of PRP for low ovarian reserve or premature ovarian failure is still considered an area of active research. Current evidence is limited, and reported outcomes vary between studies, so this option is generally discussed as part of a broader, individualized treatment strategy rather than a guaranteed solution.

For women with premature ovarian insufficiency (the current name for premature ovarian failure), egg donation — where permitted — remains the treatment with proven success, and hormone replacement therapy is important to protect bone and heart health.

This procedure is performed under general anesthesia in a hospital setting. The concentrated plasma is injected into the ovary in a process similar to egg retrieval, with the aim of supporting ovarian activity. Patients are typically advised to wait 2–3 months afterward, during which multivitamins may be recommended, before ovulation induction is considered.
