How to Get Pregnant with Twins: Causes, Methods, and Risks Explained
Curious about twin pregnancy? Learn the genetic factors, fertility treatments, and risks involved, with guidance from Dr. Najeeb Layyous.
In short: A twin pregnancy means more than one baby in the uterus in the same pregnancy. It may run in families in which more than one egg is released, and it is more likely with ovulation-stimulating drugs, insemination and IVF. It carries higher risks, including preterm birth, gestational diabetes, high blood pressure and bleeding after delivery, and no method can guarantee it.
One of the questions we hear often at the clinic is how to get pregnant with twins. It’s a topic full of curiosity and hope for many couples, so in this article we’ll walk through what’s known about twin pregnancy, how it happens, and what to expect.
It’s worth noting first that getting pregnant with twins is not simple to plan for, and a twin pregnancy usually needs closer monitoring throughout, due to the extra challenges that can come with carrying more than one baby.
What is meant by twin pregnancy?
A twin pregnancy simply means carrying more than one fetus inside the womb at the same time.
Is there a genetic factor for twin pregnancy?
Yes. Some families naturally tend to have multiple ovulations during a single menstrual cycle (meaning more than one egg is released), and this can lead to a pregnancy with more than one baby.
What are the possible complications of twin pregnancy?
- Premature birth
- The need for a cesarean section
- A higher rate of malformations compared with single pregnancies
- Gestational diabetes
- High blood pressure in pregnancy and preeclampsia
- Postpartum bleeding
- Anemia during pregnancy
Are there medical ways to get pregnant with twins?
Fertility treatments such as ovarian stimulation injections or pills do raise the chance of twins, but international guidelines consider twins a complication of treatment rather than a goal, because of the higher risks to mother and babies. For this reason these treatments are used only for couples with a fertility problem, and the doses are adjusted to aim for one healthy baby.
Is it possible to get pregnant with twins through intrauterine insemination (IUI)?
Yes. Ovarian stimulation medications are often used with IUI, which can lead to the release of more than one egg from the ovary. However, the aim is one or two mature follicles, and if too many follicles develop, the cycle is usually cancelled to avoid twins or higher-order multiples.
Is it possible to get pregnant with twins through in vitro fertilization (IVF)?
When more than one embryo is transferred to the uterus, the chance of a twin pregnancy increases. However, current guidelines recommend transferring a single good-quality embryo in most cases, and freezing the others, because this gives a similar overall chance of a baby with far fewer risks.
Is there a 100% guaranteed way to get pregnant with twins?
No method can guarantee a twin pregnancy. Ovarian stimulation drugs increase the likelihood compared to a natural cycle, but they should only be used under medical supervision for women who need them to conceive, and never just to try for twins.
What are ovarian stimulation drugs?
- Pills such as Clomid (which blocks estrogen receptors) and Letrozole (which lowers estrogen production); both make the brain release more FSH, and the twin rate is about 5–8% with Clomid and lower with Letrozole
- Hormonal injections containing the pituitary hormones LH/FSH
