Precocious Puberty in Children: Causes, Signs, and Treatment Options
Learn the causes, signs, and treatment of precocious puberty in children, and when to seek expert medical advice.
In short: Precocious puberty is the appearance of signs of puberty before the age of eight in girls and nine in boys. It may be central, due to early activation by the brain, or peripheral, due to hormones released from elsewhere. Signs are early breast growth, a growth spurt followed by short stature, and body hair. It is treated by suppressing the hormones, or by surgery if the cause is a tumour.
Precocious puberty means puberty starts earlier than expected — before age 8 in girls and before age 9 in boys. Recognizing it early can help protect your child’s health and emotional wellbeing.
During puberty, a series of natural hormonal and physical changes lead the body toward sexual maturity. When these changes begin too early, it is worth discussing with a specialist.
Why is precocious puberty worrisome?
Because it may lead to:
- Shorter adult height
- Psychological effects due to low self-esteem
There are two types of precocious puberty:
- Central precocious puberty
- Peripheral precocious puberty
Central precocious puberty:
- Early activation of the hypothalamic-pituitary-gonadal axis
- The brain sends signals leading to early stimulation of the ovaries
- Can be caused by:
- Brain injury
- Infection
- Tumors
- Genetic causes
- Head trauma
- Idiopathic (no identifiable cause)
Peripheral precocious puberty:
- Early production of sex hormones from sources outside the central nervous system
- Causes:
- Gonadal tumors or adrenal tumors
- Hormonal conditions such as ovarian cysts, congenital adrenal hyperplasia, severe underactive thyroid, or accidental exposure to hormone creams, gels or medicines used by adults
- Genetic disorders
Signs of precocious puberty:
- Early development of secondary sexual characteristics, like breast development
- Accelerated growth followed by early closure of the growth plates, which can lead to shorter adult height
- Armpit or pubic hair [* Menstruation before about 10 years of age in girls
Diagnosis:
- Physical exam revealing early sexual development
- Blood tests to measure pituitary and ovarian hormone levels
- MRI to check for structural abnormalities in the brain or organs
- X-rays of the wrist to assess bone maturity
Treatment:
- Central precocious puberty: medications to suppress the hormones released from the brain
- Peripheral precocious puberty: surgery for tumors, or medications to block excess hormone production
- In cases linked to genetic conditions, genetic counselling is recommended
It is important to address this condition as early as possible, as it can affect both the physical growth and the emotional wellbeing of children and their families.
